LICENSED CLINICAL SOCIAL WORKER

The Licensed Clinical Social Worker (LCSW) will provide quality clinical mental health consultation to providers and staff as requested; and provide comprehensive diagnostic mental health assessments, interventions, education, referrals and consultations for patients in the primary care setting. The LCSW will work within our clinic as a member of the primary care provider team and will offer consultation services to primary care providers and patients concerning behavioral changes to improve patient health status. Our Mental Health Integration (MHI) model’s interventions are brief, understandable, and effective. The LCSW will also assist in the management of the LCH Behavioral Health Program. The LCSW may work at multiple LCH sites.
